Before you can log into the on-line FY 2008 Reconciliation Application, you must complete HRSA Security Training, sign the Rules of Behavior e-mailed to you after you have completed the training, and fax the signed Rules of Behavior to 301-443-1879. Your hospital's temporary user name and password to log in to the on-line FY 2008 Reconciliation Application will be e-mailed to you. The password must be changed the first time you log in.

Affiliation agreements, supporting documentation and Medicare Cost Report (MCR) worksheets may be e-mailed as Acrobat/pdf documents to SLinney@hrsa.gov or sent by mail with HRSA Forms 99 and 99-3, which must be submitted on paper.
