of Claim Forms for Bankruptcy Claim Submissions
HEAL Lender Policy Memorandum L-2008-4
On December 1, 2007 a revised Proof of Claim form (B10) was posted at the United States Court’s website www.uscourts.gov. We encourage your legal representatives to use this revised form when filing bankruptcy cases with the courts. From the U.S. Bankruptcy Courts web site, click on “Bankruptcy Forms” on the left, which will take you to a page containing a link to Bankruptcy Forms. Click on the “Part 1” link and then on the next page scroll down on the left side of the page until you get to “B10 Proof of Claim” and click on the “Form” link underneath it to get to the actual Proof of Claim form. The direct link to the form is: http://www.uscourts.gov/rules/BK_Forms_1207/B_010_1207f.pdf
Although some courts may accept the previous version of this form, we encourage you to cease your use of all previous B10 forms in favor of the revised December 2007 version.
